A molecule name, a brand name and a research catalogue entry are different identifiers. A regulator’s decision about one finished medicine cannot be transferred to every powder, blend or alternative formulation using the same molecule name. Which comparison has direct clinical evidence?
SURMOUNT-5 directly compared tirzepatide with semaglutide in 751 adults with obesity but without type 2 diabetes. The randomized, open-label study reported mean weight changes at 72 weeks of −20.2% and −13.7%, respectively. Gastrointestinal events were the most common adverse events in both groups. That is evidence for the comparison actually tested, with its selected participants, regimens and follow-up. It is not evidence that the same difference will occur in every population or that retatrutide would occupy a particular position in that study. Open-label means participants and investigators knew the assigned treatment; it is one design feature to consider when interpreting a trial.

The EMA records cited here concern specific authorized medicines. The US FDA statement about retatrutide concerns a different molecule and a different jurisdiction.
